The Athlete Health Organization (AHO) will be offering their FREE Physical Event on Sunday, June 10th at the Wells Fargo Center. Click below for the packet which should be completed and turned in to Mr. Otarola by Friday, May 18th. Anyone attending this event MUST fill out this packet completely in order to be screened (incomplete packets will be turned away). Walk-ins are no longer allowed; you must schedule an appointment if you want to attend. In addition, this packet MAY NOT be used for any other physical examinations anywhere outside the AHO Event.  For students obtaining their own physicals, the general PIAA CIPPE Form must be used, which can be found here.
